Antrochoanal polyps are basically cystic lesions arising from the maxillary sinus and coming out of the accessory ostium and presenting as a large polyp in the nasopharynx, through the choana. It is mostly congenital and often bilateral. It is basically a disease of the first and second decades.
Treatment is complete removal including the antral cyst and its pedicle. Combined antroscopy and transnasal endoscopic approach is useful if the antral porition is not removed completely.
